IMMUNIZATION REQUIREMENTS FOR ALL JR HIGH STUDENTS

The State of Illinois immunization requirements for
ALL students in grades 6-8

Meningicoccal Vaccine
Any child entering the 6th  grade shall show proof of having received one dose of meningococcal conjugate vaccine on or after the 10th birthday. 

Tdap/ Whooping Cough- required for all junior high students
All children entering 6th through 12th grade are required to show proof of receiving one dose of Tdap, (defined as tetanus, diphtheria, acellular pertussis) vaccine regardless of the interval since the last DTaP, DT or Td dose. Tdap is approved for use in children age 11 and older.

Students in grade 6th through 12th MAY BE EXCLUDED for failure to meet these requirement.

Walk-in childhood immunizations clinics are offered Monday -Thursday 8a-11a & 1p- 5p,  Medicaid and All Kids accepted. A $10.00 fee is requested for the administration of vaccines, however, immunizations are not denied due to financial limitations.
