Registration Requirements

Below is a list of the requirements to register your student.

• Certificate of Good Standing - ISBE 33-78 - Illinois State Board of Education Transfer Form - if transferring from a public school within the state of Illinois. No school district is required to admit a new student who is transferring from another Illinois school district unless he or she can produce the standard form from the student's previous school district.

• Two Proofs Of Residency -  Both forms of identification need to include your name and address.  One form must be one of the following: driver's license or state ID. The second form must be one of the following: current monthly utility bill, phone or credit card bill, or paycheck stub.  Registration will not be allowed without absolute proof of residence regardless of the student's previous attendance in the district. 

• Birth Certificate - All students enrolling in the district for the first time are required to present a County issued original (hospital copy not accepted) birth certificate.

• Medical Information - current copies signed by an Illinois physician

  • Physical - Required for all new students by the first day of the school year. If coming from out of state, you have 30 days to obtain a physical from Illinois. Required for first-year Preschool students, Kindergarten, 6th grade and ALL new students from out of state.  Click here for the school physical form.

  • Dental - Required for Kindergarten, 2nd grade and 6th grade students.  Click here for the dental form.

  • Vision - Required for all students entering an Illinois school for the first time, including Kindergarten.  The eye exam must be completed by a licensed eye doctor, optometrist or ophthalmologist within one year of the child's entry to kindergarten.  Click here for the vision form.

• Current IEP or any other Special Education records (if applicable)

• Custody Papers (if applicable)

Registration Fees

Below is a list of potential fees:

• Registration fee          $75.00

• P.E. Uniform & Lock (Lincoln) $20.00

• Band Instrument Rental Fee $35.00

Registering your NEW student 

for the 24-25 school year

Parents/Guardians of students who wish to register their student for the 24-25 school year must follow the steps below.  Once you have completed the steps below, a school secretary will reach out to you.

1.   Complete this LaSalle Elementary 2024-2025 Registration Form.

2.   Provide proof of residency in LaSalle, Illinois.  Click here to upload 2 proofs of residency. 

3.   All students enrolling in the district for the first time are required to present a County issued original birth certificate (hospital copy not accepted).  Click here to upload a picture of your child's certified birth certificate.

4.  Please complete this health form for each new student.
