Registration Requirements
Below is a list of the requirements to register your student.
Certificate of Good Standing - ISBE 33-78 - Illinois State Board of Education Transfer Form - if transferring from a public school within the state of Illinois. No school district is required to admit a new student who is transferring from another Illinois school district unless he or she can produce the standard form from the student's previous school district.
Two Proofs Of Residency - Both forms of identification need to include your name and address. One form must be one of the following: driver's license or state ID. The second form must be one of the following: current monthly utility bill, phone or credit card bill, or paycheck stub. Registration will not be allowed without absolute proof of residence regardless of the student's previous attendance in the district.
Birth Certificate - All students enrolling in the district for the first time are required to present a County issued original (hospital copy not accepted) birth certificate.
Medical Information - current copies signed by an Illinois physician
Physical - Required for all new students by the first day of the school year. If coming from out of state, you have 30 days to obtain a physical from Illinois. Required for first-year Preschool students, Kindergarten, 6th grade and ALL new students from out of state. Click here for the school physical form.
Dental - Required for Kindergarten, 2nd grade and 6th grade students. Click here for the dental form.
Vision - Required for all students entering an Illinois school for the first time, including Kindergarten. The eye exam must be completed by a licensed eye doctor, optometrist or ophthalmologist within one year of the child's entry to kindergarten. Click here for the vision form.
Current IEP or any other Special Education records (if applicable)
Custody Papers (if applicable)
Pre-Registering your NEW student
for the 24-25 school year
Parents/Guardians of students who wish to pre-register their student for the 24-25 school year must follow the steps below. You will be notified when registration opens in TeacherEase for the 24-25 school year and at that time you will complete regular registration. If your student already attends LaSalle Elementary, you do NOT need to pre-register! This is for students NEW to LaSalle Elementary Schools.
1. Complete this LaSalle Elementary 2024-2025 Pre-Registration Form.
2. All students enrolling in the district for the first time are required to present a County issued original (hospital copy not accepted) birth certificate. Click here to upload a picture of your child's certified birth certificate.
PreK students will also need to schedule a L.E.A.S.E. screening. Please call 815-433-6433 to arrange a screening.
A school secretary will contact you once the above steps have been completed.
(Kindergarten is full day, but if you are in need of half-day kindergarten, please call the school.)
Registering your RETURNING student
for the 24-25 school year
Parents/Guardians of students who wish to pre-register their student for the 24-25 school year must follow the steps below. 1. Login to the TeacherEase app or TeacherEase website (www.teacherease.com)
2. Complete the online registration steps.
